How to Send Us Your Handpieces

Below are two items you will need to print out to get your handpiece to us. You may click on each item to open them off the site as a PDF, or you can “Right Click” and “Save As” to save them to your computer for printing. Both PDF’s below are “fillable,” meaning you can use your keyboard and mouse to fill in the information.

Print and fill out the Work Order Form, then place inside the shipping box.

You may click on each item to open on your screen, or you can right-click and choose “Save As” in order to save to your computer for printing.

Please print this out to give us an idea of what is wrong; what you would like done; and if you prefer a free estimate beforehand. We recommend keeping a copy for your records.
